To facilitate the process, please be prepared to provide the following information:

• Full name
• New address
• Phone number
• Driver’s license number
• Date of occupancy
• Ownership status (own or rent)
• If renting, the name of your landlord or property management personnel
• Landlord/Property Manager phone number
• Landlord/Property Manager email address
• Lease agreement
